Application Deadline for the Vancouver Program

Module Two: April 15, 2009
Module Three: June 20, 2009

If this is your first Module, a $500.00 nonrefundable deposit is required within 1 week of program acceptance.  Full payment for all participants is required by May 5, 2009.

To enroll, prospective applicants must submit:

• Modular Application Form
• Two Professional Reference Letters

The above forms may be completed directly on the document on your computer, and mailed to info@labancan.org electronically.

Fees

A one-time $100.00 USD fee is required when applying. Click here to pay application fee online

The fee for each Module is $2,250.00 USD. Click here to pay tuition online

Payment in full for each module is to be paid to LSSI prior to the beginning of each module. Late application/enrolment may be possible but a late fee will be assessed.

Payment method may include personal cheque, money order, or credit card.
